End-of-Life Planning

End-of-life planning involves a range of important tasks aimed at ensuring a person’s wishes are respected and their affairs are in order as they approach the final stage of life. These tasks include supporting in the completion of documents such as advance care plans and directives, and powers of attorney to outline medical and financial preferences. It also involves making decisions about healthcare, funeral arrangements, and the distribution of assets.

Communicating these plans with loved ones and healthcare providers is essential to avoid confusion and reduce stress for family members. Additionally, organizing important documents, reviewing insurance policies, and addressing digital legacies can help ensure a smoother transition and peace of mind for everyone involved.

As an End of Life Doula (not a lawyer) Cory can help you get organized, methodically work through all the steps, and have those challenging conversations with family members and loved ones.

Advance Care Planning

In British Columbia, creating an Advance Care Plan is essential for ensuring that your preferences regarding life-prolonging treatments are respected if you become incapable of making decisions. This plan allows you to express your values and wishes concerning medical interventions, such as resuscitation or artificial nutrition, and to appoint a representative—known as a substitute decision-maker—who can make health care decisions on your behalf.

Without such a plan, decisions may default to individuals who may not be fully aware of your desires, potentially leading to treatments that do not align with your values. By documenting your choices and designating a trusted representative, you provide clear guidance to your loved ones and health care providers, reducing uncertainty and emotional burden during critical times.

End of Life Doula Cory Bretz can walk you through the process of creating your Advance Care Plan, legally designating your representatives, and communicating your wishes to your family and loved ones.
